I figured it out. I used the following code,

<parameterGroup>
    <name>CustomDialog</name>
    <title>Custom Dialog</title>
    <explanation></explanation>
    <value>CustomDialog</value>
    <default></default>
    <parameterList>
        <booleanParameter>
            <name>Checkbox1</name>
            <description>Checkbox 1</description>
            <explanation></explanation>
            <value>1</value>
            <default>1</default>
            <displayStyle>checkbox-left</displayStyle>
        </booleanParameter>
        <booleanParameter>
            <name>Checkbox2</name>
            <description>Checkbox 2</description>
            <explanation></explanation>
            <value>1</value>
            <default>1</default>
            <displayStyle>checkbox-left</displayStyle>
        </booleanParameter>
        <booleanParameter>
            <name>Checkbox3</name>
            <description>Checkbox 3</description>
            <explanation></explanation>
            <value>1</value>
            <default>1</default>
            <displayStyle>checkbox-left</displayStyle>
        </booleanParameter>
    </parameterList>
</parameterGroup>
